The bigger concept
'First' in a communication item almost always means assess before informing. Asking the client what they expect and value opens the conversation, reveals fears and misconceptions, and keeps the care plan client-centered. Launching into hospice logistics, offering reassurance, or deferring entirely to the provider skips that step. The PN/VN relays what the client shares to the RN so the team can plan palliative or hospice support.
