Why this is the answer

The bigger concept
Confidentiality items test whether you release information only to people the client has authorized and only after their identity is verified. A phone caller cannot be verified by voice, so the PN/VN does not confirm the client is even on the unit. Routing the call to the charge nurse, who can check the client's authorized-contact list and any password system, protects the client and the nurse.
