Data collection includes psychosocial stressors, not just physical findings. Infertility affects identity, relationships, and finances, and clients rarely volunteer it, so a neutral, nonjudgmental response when it surfaces is key. The PN/VN documents the stressor and reports it so the RN can plan support and referrals.
