Pediatric Amoxicillin Dose Calculation (mg/kg/day)

Physiologic Needs · Question from the ATI PN Fundamentals 2023 Proctored Exam study set

Question: A nurse is preparing to administer amoxicillin 50 mg/kg/day in 4 divided doses to a child who weighs 55 lb. The available concentration is 125 mg/5 mL. How many mL should the nurse administer with each dose?
12.5 mL

Correct answer: 12.5 mL

Why this is the answer

Step 1 — convert weight: 55 lb ÷ 2.2 = 25 kg. Step 2 — daily dose: 25 kg × 50 mg/kg/day = 1,250 mg/day. Step 3 — per-dose amount: 1,250 mg ÷ 4 doses = 312.5 mg/dose. Step 4 — volume: 312.5 mg ÷ 125 mg × 5 mL = 12.5 mL per dose. Each step carries its units so that mg and kg cancel, leaving mL as the final answer.

The bigger concept

Weight-based pediatric calculations follow the same four steps every time: convert pounds to kilograms, multiply by the mg/kg/day order, divide by the number of doses, then convert the mg dose into mL using the available concentration. Units should cancel at every step. Always round only at the end, and compare the final dose against a safe-dose reference before administering.
