Instilling Ear Drops in an Adult: Dropper Position

Physiologic Needs · Question from the ATI PN Fundamentals 2023 Proctored Exam study set

Question: A nurse is preparing to instill otic medication for an adult client. Which action should the nurse take?
Hold the ear dropper 1 cm (0.5 in) from the client's ear

Correct answer: Hold the ear dropper 1 cm (0.5 in) from the client's ear

Why this is the answer

Holding the dropper close to, but not touching, the ear canal prevents contamination of the dropper and injury to the canal while still delivering the medication accurately.

The bigger concept

Otic administration steps: warm the drops to room temperature, position the client on the unaffected side, straighten the canal by pulling the pinna up and back for adults (down and back for children under 3), hold the dropper about 1 cm above the canal without touching it, instill along the side of the canal, and have the client stay side-lying for 2–3 minutes. A cotton ball placed loosely in the outer ear is optional.
