Question: A newly licensed nurse is placing an indwelling urinary catheter for a female client. Which statement indicates a need for further teaching?

Why this is the answer

After urine return, the catheter should be advanced only an additional 1–2 inches (2.5–5 cm), not 4 inches, to secure balloon placement within the bladder without unnecessary insertion.

Indwelling catheter insertion is a sterile procedure with numeric details the exam tests directly: insert 2–3 inches (5–7.5 cm) in a female client and 6–8 inches (15–20 cm) in a male until urine flows, then advance another 1–2 inches before inflating the balloon so the balloon is fully inside the bladder. Inflating too early injures the urethra. Secure the catheter to the thigh and keep the drainage bag below bladder level.
