Each of these reflects a return toward normal fluid and electrolyte balance: near-normal labs, moist mucosa, elastic turgor, adequate light-yellow urine output, and more formed stool all indicate rehydration and resolution of diarrhea.
The bigger concept
Evaluation items ask you to compare findings to the expected outcome. Rehydration shows up as urine output of at least 30 mL/hr that is light yellow, moist mucous membranes, skin that snaps back, stable vital signs, sodium and potassium back in range, and stool that is forming again. Persistent tachycardia, dark concentrated urine, or a rising sodium would mean the plan is not working and must be reported.
