Ostomy teaching items focus on the details that prevent leakage and peristomal skin breakdown. The barrier opening is cut about 1/8 inch larger than the stoma, the skin is cleaned with warm water and dried completely, and the barrier is pressed and held so body heat molds it to the skin. The pouch is emptied when one-third to one-half full. The PN/VN also teaches the client to report a stoma that turns dark, dusky, or purple.
