What the 2023 ATI RN Nutrition proctored exam covers
ATI organizes Nutrition around five content areas. Our 60-question practice exam distributes questions across them in a similar proportion to what you'll see on test day, predominantly classic four-option multiple choice with a smaller number of select-all-that-apply items, matching the 2023 series format.
Basic Nutrition Principles & Nutrients
The RN Nutrition exam tests core concepts like macronutrients, micronutrients, fluid and electrolyte balance, and how to read a nutrition label or diet order accurately. Expect questions on vitamin/mineral deficiencies and toxicities, and which foods are good sources of specific nutrients.
Nutrition Across the Lifespan
Nutritional needs shift across infancy, childhood, pregnancy/lactation, and older adulthood. Expect questions on infant feeding and introduction of solids, increased caloric and protein needs in pregnancy, and age-related risks like decreased thirst sensation and reduced caloric needs in older adults.
Therapeutic & Modified Diets
This is one of the heaviest-weighted domains: diabetic diets, renal diets (sodium/potassium/phosphorus/protein restriction), cardiac diets (sodium and fat restriction), low-residue and high-fiber diets, and dysphagia diet levels. You'll need to match a diagnosis to the correct therapeutic diet and identify foods to include or avoid.
Enteral & Parenteral Nutrition
Covers indications for enteral (tube) feeding versus parenteral (IV) nutrition, tube placement verification, aspiration precautions, and complications like refeeding syndrome and dumping syndrome. These questions often test priority nursing actions when a complication is suspected.
Drug-Nutrient Interactions & Nutritional Status
Certain medications interact with specific nutrients or foods — warfarin and vitamin K, MAOIs and tyramine, and grapefruit juice with several drug classes are classic exam topics. Also expect questions on recognizing signs of malnutrition, obesity-related risk factors, and basic nutrition support for wound healing.
Sample questions from the 2023 RN Nutrition exam
Rationale: Green leafy vegetables like spinach are high in vitamin K, which can affect warfarin's anticoagulant effect. Clients are taught to keep vitamin K intake consistent day to day rather than eliminate it, since sudden changes affect INR more than a steady intake.
Rationale: Clients with chronic kidney disease typically need restricted sodium, potassium, and phosphorus to reduce the workload on the kidneys and prevent electrolyte imbalances, along with individualized protein and fluid limits rather than unrestricted intake.
Rationale: Abdominal distention and nausea in a client on tube feedings suggest possible intolerance or delayed gastric emptying. The nurse should first assess bowel sounds and gastric residual volume before making any changes to the feeding, rather than increasing the rate or discontinuing feeding without further assessment.
