What the ATI Maternal-Newborn proctored exam actually covers

ATI organizes Maternal-Newborn around five content domains. Here's what each one tests and how to study for it.
1

Antepartum Care & High-Risk Pregnancy

Covers routine prenatal care plus high-risk conditions: preeclampsia/eclampsia, gestational diabetes, placenta previa, and abruptio placentae. Know the hallmark differentiators ATI loves to test — painless bright-red bleeding points to placenta previa, while painful dark bleeding points to abruptio placentae — and the priority intervention for each (magnesium sulfate prevents seizures in preeclampsia; it does not lower blood pressure).

2

Intrapartum Care & Labor/Delivery

Focuses on fetal heart rate monitoring, stages of labor, and pain-management options. The VEAL CHOP mnemonic (Variable = Cord compression, Early = Head compression, Accelerations = Okay, Late = Placental insufficiency) is the single most tested framework in this domain — expect a strip-interpretation question that hinges on matching a deceleration pattern to its cause and nursing response.

3

Postpartum Care & Complications

Fundal assessment and hemorrhage prevention dominate this domain. A boggy or displaced fundus is the classic scenario — before assuming uterine atony, rule out a full bladder, since a distended bladder is a common and easily-corrected cause of fundal displacement. Also expect questions on lochia progression, breastfeeding physiology, and postpartum mood disorders versus baby blues.

4

Newborn Assessment & Care

Apgar scoring at 1 and 5 minutes is guaranteed to appear — know the five components (heart rate, respiratory effort, muscle tone, reflex irritability, color) and their 0–2 point scale cold. Beyond Apgar, expect thermoregulation, gestational-age assessment, and priority monitoring for at-risk newborns, including infants of diabetic mothers who are watched closely for hypoglycemia.

5

Medication Safety in Pregnancy & Lactation

Covers which medications are safe, contraindicated, or require monitoring during pregnancy and breastfeeding, plus core OB pharmacology: Rho(D) immune globulin (RhoGAM) for Rh-negative mothers, magnesium sulfate for preeclampsia, betamethasone for fetal lung maturity, and oxytocin for labor induction or postpartum hemorrhage control.

Sample ATI Maternal-Newborn questions with rationales

These are five real questions from our 42-question Maternal-Newborn bank — the same format and difficulty you'll see in the full quiz engine, with the correct answer and rationale shown.
A nurse is caring for a client at 32 weeks gestation who reports sudden, painless vaginal bleeding with bright-red blood and a soft, nontender abdomen. Which condition should the nurse suspect?
A. Abruptio placentae
B. Placenta previa
C. Ectopic pregnancy
D. Preterm labor

Rationale: Placenta previa classically presents with painless, bright-red bleeding and a soft abdomen, while abruptio placentae presents with painful bleeding and uterine rigidity.

During continuous fetal monitoring, a nurse observes recurrent late decelerations that return to baseline after each contraction ends. What is the nurse's priority action?
A. Document the finding and continue routine monitoring
B. Reposition the client to the left lateral position and increase the IV fluid rate
C. Prepare the client for immediate forceps delivery
D. Administer a tocolytic agent

Rationale: Late decelerations reflect uteroplacental insufficiency; repositioning to improve venous return and increasing IV fluids is the first-line intervention to improve placental perfusion.

On postpartum day 1, a nurse finds a client has saturated a perineal pad within 15 minutes with bright-red blood, but the fundus is firm and midline at the level of the umbilicus. What should the nurse do first?
A. Vigorously massage the fundus
B. Check the client's vital signs and notify the provider
C. Have the client ambulate to the bathroom
D. Document the finding and reassess in one hour

Rationale: Heavy bleeding with a firm fundus points to a laceration rather than uterine atony; massage will not help, so the nurse should assess for shock and notify the provider promptly.

A newborn at 1 minute of life has a heart rate of 150/min, a strong cry, active motion, a grimace in response to suction, and a completely pink body. What is the Apgar score?
A. 7
B. 8
C. 9
D. 10

Rationale: Heart rate >100 (2) + strong respiratory effort (2) + active tone (2) + grimace-only reflex response (1, not the full 2 reserved for a vigorous cry/cough) + pink color (2) totals 9.

A client at 29 weeks gestation in preterm labor is ordered a course of betamethasone. What is the primary purpose of this medication?
A. To stop uterine contractions
B. To accelerate fetal lung maturity
C. To prevent neonatal infection
D. To lower maternal blood pressure

Rationale: Betamethasone crosses the placenta and stimulates fetal surfactant production, accelerating lung maturity and reducing the risk of neonatal respiratory distress syndrome.

ATI Maternal-Newborn proctored exam FAQ

1

How many questions are on the ATI Maternal-Newborn proctored exam?

ATI's official Maternal-Newborn proctored exam typically runs 60–70 scored multiple-choice and Next Generation NCLEX-style items, though your nursing program sets the exact count, time limit, and number of unscored pretest items — check your course syllabus or ATI Learning System for your cohort's specific format.

2

What score do I need to pass ATI Maternal-Newborn?

ATI reports results as proficiency levels (Level 1, 2, or 3) rather than a single passing percentage, and each nursing program sets its own required level for the course. Ask your instructor what level your program requires before test day.

3

Is this the same test as ATI's official proctored exam?

No. This is an independent, original study resource written to cover the same content areas and question style as ATI's Maternal-Newborn content-mastery exam. It is not affiliated with, endorsed by, or a reproduction of Assessment Technologies Institute's proprietary exam content.

4

How is this different from the practice assessments inside my school's ATI account?

Your school's ATI Learning System practice assessments are the official ones and should be your primary prep. This site is a free, unlimited supplement — one question at a time with an instant rationale, no login or institutional access required — useful for extra reps between official practice attempts.

5

What should I study first if I'm running out of time?

Prioritize fetal heart rate interpretation (VEAL CHOP) and postpartum hemorrhage priorities above — both appear repeatedly in slightly different clinical wrappers, so mastering the underlying pattern pays off across many questions at once.
