What the 2026 ATI RN Fundamentals proctored exam covers

The 2026 Fundamentals content mastery exam is built around five domains. Our 70-question practice exam distributes questions across them in roughly the same proportion you'll see on test day. The 2026 series leans harder on Next Generation NCLEX (NGN) item types — extended multiple response, drag-and-drop ordering, and clinical-judgment case studies — so our 2026 exam includes those formats alongside classic multiple choice.

Foundations of Practice & Clinical Judgment (≈ 20 questions)

Nursing process, prioritization (ABCs, Maslow, acute-before-chronic), delegation and scope of practice, and "first action" scenarios. This is the largest block on the 2026 exam and the one where most points are lost — every question in this section of our exam asks you to choose between plausible right answers, not spot an obviously wrong one.

Safety & Infection Control (≈ 15 questions)

Standard and transmission-based precautions, PPE sequence, fall and restraint safety, fire response (R.A.C.E./P.A.S.S.), medication rights and error prevention. Procedural and sequence-based — high-yield, fast points once memorized.

Health Assessment & Documentation (≈ 12 questions)

Normal versus abnormal findings, vital-sign interpretation, pulse and edema grading scales, objective documentation, and SBAR hand-off. Questions give you a raw finding and ask what it means or how to chart it.

Physiologic Needs (≈ 13 questions)

Hygiene, mobility and positioning, elimination, sleep, nutrition and hydration, oxygenation basics, and skin integrity. Includes lab-value interpretation such as albumin and pre-albumin for nutritional status.

Psychosocial Needs (≈ 10 questions)

Therapeutic communication, culturally competent care, coping, grief and loss, client rights, confidentiality, and informed consent. Watch for answers that sound kind but are not client-centered.

Rationale: Airborne precautions (e.g., tuberculosis, measles, varicella) require a fit-tested N95 respirator; a surgical mask is used for droplet precautions.

A nurse is documenting a client's pedal edema and notes an indentation of about 4 mm that rebounds in 10–15 seconds. How should this be recorded?

How is the 2026 exam different from the 2023 exam?

The 2026 content mastery series updates the Fundamentals blueprint and adds more Next Generation NCLEX (NGN) item types — case studies, extended multiple response, and ordering items. Our 2026 exam mirrors that format. If your program still uses the 2023 series, choose the 2023 exam instead.

How many questions are on the ATI Fundamentals 2026 proctored exam?

ATI's official exam typically runs 60–70 scored items plus a small number of unscored pretest items, so a 70-question practice exam gives you a realistic full-length run. Your program sets the exact count and time limit — check your syllabus.
