ATI Dosage Calculation Practice: Common Problem Types and How to Solve Them
Dosage calculation questions show up on nearly every ATI proctored exam, regardless of topic — and they're often where students lose points they'd otherwise have earned. Here's a breakdown of the problem types you'll actually see, with the reasoning behind each one.
Basic dosage calculation (desired over have)
The most common format: you're given a prescribed dose and the concentration available, and asked how many mL or tablets to administer. The formula is straightforward — desired dose ÷ dose on hand × quantity — but errors usually come from unit mismatches (mg vs. mcg) rather than the math itself. Always convert units before you calculate, not after.
IV flow rate calculations
These ask you to determine mL/hr or gtt/min for an IV infusion. The two things students mix up most: whether the pump is set in mL/hr (most modern infusion pumps) versus a manual gravity drip requiring gtt/min based on the tubing's drop factor. Read the question carefully for which one is being asked — the setup looks similar but the formulas differ.
Weight-based dosing
Common with pediatric medications and some high-alert drugs, these questions give you a dose per kg (or per lb) and require you to calculate the client's weight in the correct unit first. A frequent trap: the client's weight is given in pounds, but the drug reference is per kg — convert weight first, then calculate the dose.
Titration and dose-adjustment problems
Slightly more advanced items ask you to adjust an existing infusion rate based on a client response (e.g., titrating a vasoactive drip to a target blood pressure). These test whether you understand the clinical reasoning behind the number, not just the arithmetic — ATI proctored exams increasingly favor this style over plain plug-and-chug math.
A few habits that prevent most errors
- Write out every unit at each step — don't calculate in your head
- Convert all values to the same unit system before doing the math
- Double-check whether the question wants mL/hr, gtt/min, mg, or a tablet count — they're not interchangeable
- Sanity-check your final answer: does the dose seem clinically reasonable?
