What the 2026 ATI RN Mental Health proctored exam covers

Therapeutic Communication & the Nurse-Client Relationship

Expect questions built around a client statement or behavior, asking you to pick the most therapeutic response. The pattern ATI rewards: reflect feelings, use open-ended questions, and avoid arguing with a client's perception — with a hallucinating client, you acknowledge their experience without confirming the hallucination itself ("I don't hear the voices, but I understand you do."). Responses that are dismissive, confrontational, or that agree with a delusion are almost always wrong.

Mood, Anxiety & Psychotic Disorders

This is the largest content area: major depressive disorder, bipolar disorder, generalized anxiety, OCD, PTSD, and schizophrenia-spectrum disorders. Questions test recognizing symptom clusters, matching interventions to the acute phase of an illness (a manic client needs finger foods and a low-stimulation environment, not group therapy), and knowing which medication class treats which disorder.

Crisis Intervention & Safety

Suicide risk assessment is the single highest-yield topic here. When a client makes a statement suggesting hopelessness, the priority action is almost always to directly assess lethality — ask if they have a plan — before documenting, notifying family, or medicating. Also expect de-escalation technique questions and criteria for one-to-one observation or a safe environment.

Substance Use Disorders

Know the withdrawal syndromes and their nursing priorities: alcohol withdrawal (seizure risk, CIWA scoring, thiamine to prevent Wernicke-Korsakoff syndrome), opioid withdrawal (uncomfortable but rarely life-threatening), and stimulant withdrawal (depression, fatigue). Questions often test whether you recognize which withdrawal is a medical emergency versus which is simply distressing.

Legal & Ethical Issues in Psychiatric Care

Voluntary vs. involuntary admission status changes what rights a client retains — a voluntarily admitted client can still refuse psychotropic medication in most cases, while involuntary status limits the right to leave but does not remove the right to informed consent. Confidentiality, duty to warn, and least-restrictive-environment principles round out this domain.
