Study Guide

What the 2026 ATI PN/VN Medical-Surgical proctored exam covers

ATI organizes Medical-Surgical around major adult health conditions, scoped to what a PN/VN performs and reports. Our 94-question practice exam distributes questions across them in a similar proportion to what you'll see on test day. The 2026 series leans harder on Next Generation NCLEX (NGN) item types — extended multiple response, case studies, and select-all-that-apply — so our 2026 exam includes 9 select-all-that-apply and 35 case-study/NGN items alongside classic multiple choice.

1

Cardiovascular & Respiratory Care

Expect questions on heart failure, MI, dysrhythmia recognition, oxygen therapy, and COPD/asthma management at the PN/VN scope. A PN/VN monitors vital signs and oxygen saturation, reports significant changes, and performs routine interventions like positioning and oxygen delivery — but titrating vasoactive drips or interpreting new dysrhythmias for treatment decisions is RN- or provider-level judgment. Know which cardiac and respiratory findings require an immediate report versus routine documentation.

2

Endocrine & Metabolic Disorders

Diabetes management (insulin administration, hypoglycemia/hyperglycemia recognition, foot care teaching) and thyroid disorders are common here. PN/VN items focus heavily on recognizing signs of hypoglycemia versus hyperglycemia and knowing the immediate action (usually a fast-acting carbohydrate) rather than adjusting insulin regimens, which stays with the RN or provider.

3

Gastrointestinal & Renal Care

This domain covers GI bleeding, bowel obstruction, ostomy care, and renal failure/dialysis basics. Expect intake-and-output tracking, NPO status, and routine ostomy or catheter care questions — tasks well within PN/VN scope — alongside items that test when an abnormal finding (like sudden decreased urine output) needs to be reported up rather than managed independently.

4

Musculoskeletal & Neurological Care

Fractures, joint replacements, stroke, and neuro checks show up frequently. PN/VN questions often test proper positioning, cast/traction care, and basic neuro assessment (LOC, pupil checks, grip strength) as data collection — with escalation to the RN when findings suggest a new or worsening neurological deficit.

5

Perioperative & Wound Care

Pre- and post-operative teaching, dressing changes, wound assessment, and infection signs make up a large share of Medical-Surgical content. These are hands-on, procedural items with a clear correct sequence or precaution level, making them efficient points to lock in with focused review of aseptic technique and post-op complication recognition (e.g., signs of DVT, atelectasis, or wound dehiscence).

Sample questions from the 2026 PN/VN Medical-Surgical exam

Three questions written to the PN/VN Medical-Surgical scope, in the style of the 94-question 2026 exam, with the correct answer and rationale shown. The full PDF puts the answer key in a separate section so you can sit the exam untimed or timed first.
A client with type 1 diabetes reports feeling shaky, sweaty, and confused. The LPN/LVN checks a blood glucose of 58 mg/dL. What should the LPN/LVN do first?
A. Administer the client's scheduled insulin dose
B. Give 15-20 grams of a fast-acting carbohydrate
C. Call the provider to adjust the insulin order
D. Recheck the blood glucose again in one hour

Rationale: A blood glucose of 58 mg/dL with symptoms indicates hypoglycemia, which requires immediate treatment with a fast-acting carbohydrate. Giving insulin would worsen the hypoglycemia. Rechecking without treating first delays needed care, and adjusting insulin orders is outside PN/VN scope.

On post-operative day 2, a client's abdominal incision has a sudden increase in serosanguineous drainage and the client reports a 'popping' sensation. What should the LPN/LVN do first?
A. Apply an abdominal binder and continue routine care
B. Document the finding and reassess in four hours
C. Cover the area with a sterile saline-soaked dressing and notify the RN immediately
D. Remove the existing dressing to fully inspect the wound

Rationale: These findings suggest possible wound dehiscence, a surgical emergency. The priority is to cover the wound with a sterile saline-soaked dressing and notify the RN right away. Routine care, delayed reassessment, or fully removing the dressing to explore the wound are not appropriate first actions.

An LPN/LVN is caring for a client 6 hours after a total knee replacement. Which finding should be reported to the RN immediately?
A. Pain rated 4/10 at the surgical site
B. Mild swelling around the incision
C. Calf pain, warmth, and redness in the operative leg
D. Serosanguineous drainage on the dressing the size of a quarter

Rationale: Calf pain, warmth, and redness suggest a possible deep vein thrombosis (DVT), a serious post-operative complication that requires immediate RN notification. Mild pain, mild swelling, and small amounts of expected drainage are common post-op findings that can be documented and monitored.
