ATI Fundamentals Proctored Exam Test Bank

A free ATI Fundamentals proctored exam test bank: original practice questions with full rationales, organized by content domain, no sign-up required.

1

What's in the bank

The Fundamentals test bank covers all five ATI content domains — Foundations of Practice & Clinical Judgment, Safety & Infection Control, Health Assessment & Documentation, Physiologic Needs, and Psychosocial Needs — with a full rationale on every question. The RN bank runs 196 questions; the PN/VN bank runs 70, scoped to what a PN/VN performs versus reports. Every question includes the correct answer and the reasoning behind it, not just a bare answer key.

2

How to use a test bank effectively

Test yourself before you review, not after. Answer a question cold, check it, and read the rationale even when you got it right — the reasoning is what transfers to slightly different question wrappers on the real exam. Working straight through a bank passively, without checking rationales, is the single most common way students waste a test bank's value.

Sample questions by domain

A few questions pulled from the full bank, one per domain, with the rationale shown.
Which action is the priority for a nurse when a patient is experiencing an anaphylactic reaction?
A. Administering oxygen
B. Maintaining a patent airway
C. Administering epinephrine
D. Starting an IV line

Rationale: Maintaining an airway is the first step in the ABC (Airway, Breathing, Circulation) priority framework to prevent respiratory arrest. (Foundations of Practice & Clinical Judgment)

When removing Personal Protective Equipment (PPE), which item should the nurse remove first?
A. Gown
B. Mask
C. Gloves
D. Goggles

Rationale: Gloves are considered the most contaminated piece of PPE and should be removed first to prevent cross-contamination. (Safety & Infection Control)

When assessing a patient's peripheral pulses, the nurse notes a "3+" pulse. How should this be documented?
A. Weak and thready
B. Normal
C. Full and bounding
D. Absent

Rationale: In the standard 0–4 scale for pulses, 2+ is normal, and 3+ is considered increased, full, or bounding. (Health Assessment & Documentation)

A client who is NPO prior to a procedure asks for a few sips of water. Which response by the LPN/LVN is appropriate?
A. Give the client a few sips of water since it's a small amount
B. Allow the client to chew on ice chips instead
C. Explain that the client must remain NPO and report the request to the RN
D. Delay the procedure until the client is no longer thirsty

Rationale: NPO orders must be followed exactly to reduce aspiration risk during the procedure. Both water and ice chips count as oral intake and are not permitted. (Physiologic Needs)

Test bank FAQ

1

Is this an official ATI test bank?

No. This is an independent, original resource written to match ATI's Fundamentals content areas and question style. It is not affiliated with, endorsed by, or a reproduction of Assessment Technologies Institute's proprietary exam content. That distinction matters — platforms that sold what they claimed was real exam content have been shut down by court order; see what happened to Naxlex.

2

How many questions are there?

196 in the RN Fundamentals bank and 70 in the PN/VN Fundamentals bank today, with more added as the site grows.

3

Are rationales included?

Yes — every single question includes a rationale explaining why the correct answer is correct, not just an answer key.
